Physical Examination Form
The foreign students, who intend to study in China, should go through a physical check-up before they come to China, according to the requirements of the Physical Examination Record for Foreigners. The physical examination should be taken in the public hospitals one month before their departure, if the physical check-up is done at a private hospital, the student should get the certificate notarized. If the physical check-up is over 6 months old on the day of their registration, they should have re-checked up in the city hospital. The expenses should be covered by themselves.
How to Fill?
All the items of this form should be filled in carefully and clearly. The report should be attached with the negative film for Chest X-ray exams, and the examination certificates for laboratory exams (Serodiagnosis), which include exams on Cholera, Yellow fever, Plague, Leprosy, Venereal Disease, Opening lung tuberculosis, AIDS, Psychosis, Liver function, and HBsAg. The hospital seal should be put across the photo on the Examination Record, otherwise, the Record is considered invalid.
Where Is It required?
For Chinese universities, the Physical Examination Form is required on three different stages
- You have to send a copy of the physical examination form along with other documents at the time of admission application. At this stage, you don’t have to send all examination reports only this form is required.
- You also need a copy of the Physical Examination Form at the time of visa application at the Chinese embassy in your country. The Physical Examination Form is very important to get a Chinese visa.
- After reaching china you need the original Physical Examination Form and all examinations reports along with the Physical Examination Form. On their arrival at University, the Record (an original copy) and laboratory exam certificates should be sent to the city Bureau for check. Those whose exam is not qualified should get the physical re-check-up done in a city hospital. The expenses should be covered by themselves. The Physical Examination Record is one of the documents that are needed in the process of going through the visa formalities after reaching china. The students should bring the original copy along with them when they come to China.
In some universities, they don’t accept foreigners’ physical forms and they ask students to recheck up. After reaching China, the university send all of the new coming students to the hospital to make every test (i think it was only 50rmb or something like that). I also did it in my country, luckily for free, but I know in some countries you need to pay for it. So maybe it’s better if you send a mail to your Chinese university so they tell you what to do.
I think it’s also good to check yourself before leaving, knowing you don’t have anything serious, since you will be a long time away from home.
